Responsibilities and requirements for MDS Nurse Coordinator

  • Conduct and coordinate the development and completion of the resident assessment (MDS)
  • Maintain and periodically update written policies and procedures that implement MDS and care plan
  • Assist the resident in completing the care plan portion of the resident’s discharge plan
  • Develop and implement procedures with the Director of Nursing Services to inform all assessment team members of the arrival of newly admitted residents
  • Assist Facility directors and supervisors in scheduling the resident assessment and care plan meetings
  • Assist in determining appropriate treatment, selecting activities and exercises based on medical and social history of residents
  • Participate in the development and implementation of resident assessments (MDS) and care plans, including quarterly and annual reviews

Qualifications

  • Valid RN/LPN License in the state of VA
  • Must have at least 1 year of MDS coordinator & long-term care setting experience preferred; however willing to train the right candidate.
  • Clinical experience, education or specialty skills specific to geriatrics is desired
  • Strong knowledge of nursing and medical practices and procedures, as well as laws, regulations, and guidelines pertaining to long term care.
  • Great interpersonal skills for effective daily interaction with Residents, family members and all members of the Interdisciplinary Team
  • Must be a self-starter and a high attention to accuracy and details and possess good time management skills
  • Ability to make independent decisions and follow through with limited supervision.
  • Full vaccination against COVID-19 or a medical/religious exemption is required.

Salary/ Wage Range

Compensation for the role will depend on a number of factors, including a candidate’s qualifications, skills, competencies and experience and may fall outside of the range shown.

Schedule:

  • 12 hour
  • 8 hour
  • Weekend availability
  • Share Nursing Administration On-Call duty on nights M-F as scheduled or requested by DON or Administration.

Education:

  • Associate's
  • Bachelor's (Preferred)

Experience:

  • Skilled Nursing Home LTC: 2 years (Preferred)
  • MDS: 2 years (Preferred)

License/Certification:

  • State licensure as a RN/LPN
