Day (United States of America) The Comprehensive Stroke Center Nurse Coordinator is fundamental to the development, implementation, and evaluation of the stroke program directed towards the provision of quality services for patients with stroke and other cerebrovascular diseases from a multi-disciplinary perspective throughout the continuum of care. The CSC Nurse Coordinator will work collaboratively with the Program Director, Clinical Program Manager, physicians, trainees, and nursing staff from multiple disciplines, health care quality staff, and radiology technicians in a mutually supportive relationship to assume responsibilities for data analysis, process and performance improvement activities, and development and establishment of operational and strategic goals consistent with the overall mission of the program and as they relate to the Joint Commission and American Heart Association requirements. The CSC Nurse Coordinator will also participate in providing education and support to patients and their families in the inpatient and outpatient settings to navigate through their needs, and to individuals and groups involved in stroke care including EMS personnel.Job Description:
Primary Responsibilities:
- Collaborates with the Comprehensive Stroke Center (CSC) Program Director, Clinical Program Manager, and the multidisciplinary team on the work and projects of the program with a focus on reviewing performance and safety in order to evaluate issues, enhance clinical outcomes and provide loop closure. - Participates in providing education and support to patients and their families in the inpatient and outpatient settings to navigate through their needs. Participates in stroke-related community education and outreach efforts. (essential).
- Participates in providing education to clinical staff, and of individuals and groups involved in stroke care including EMS personnel. (essential)
- Assists in the preparation and participates in regulatory related survey visits such as the CSC bi-annual Joint Commission accreditation survey. Plays a key role in compiling required supporting documents and data associated with the CSC program to meet Joint Commission and other regulatory guidelines (essential)
- Identifies quality improvement needs and develop performance improvement activities within the CSC program and in collaboration with the referring agencies. (essential)
- Performs 7 day and 90 day post discharge follow up calls with patients and/or families. Assesses patient and families perception of stroke-related care. (essential)
- Maintains and updates appropriate tracking system to monitor and analyze patient outcomes and develops files and data systems to ensure optimal care is given. Develop and maintain complex data sets in partnership with the Department of Healthcare Quality. (essential)
- Participate and co-leads a variety of CSC Committee meetings and special projects (essential)
Required Qualifications:
- Bachelor’s degree in Nursing required.
- License Registered Nurse required, and Certificate 1 Basic Life Support required.
- 3-5 years related work experience required.
- Advanced skills with Microsoft applications which may include Outlook, Word, Excel, PowerPoint or Access and other web-based applications. May produce complex documents, perform analysis and maintain databases.
Preferred Qualifications:
- Prior experience with stroke, Neurology or neurosurgical patients

Written Communications: Ability to communicate clearly and effectively in written English with internal and external customers.
Oral Communications: Ability to comprehend and communicate complex verbal information in English to medical center staff, patients, families and external customers.
Knowledge: Ability to demonstrate full working knowledge of standard concepts, practices, procedures and policies with the ability to use them in varied situations.
Team Work: Ability to lead collaborative teams for larger projects or groups both internal and external to the Medical Center and across functional areas. Results have implications for the management and operations of multiple areas of the organization.
Customer Service: Ability to provide a high level of customer service to patients, visitors, staff and external customers in a professional, service-oriented, respectful manner using skills in active listening and problem solving. Ability to remain calm in stressful situations.

Physical Nature of the Job:
Light work: Exerting up to 20 pounds of force frequently to move objects. Some elements of the job are sedentary, but the employee will be required to stand for periods of time or move through out the hospital campus
