Description:Provides assessment and evaluation of service recipients as it pertains to their emotional and physical well-being. Ensures compliance with all governmental health regulations as they pertain to scope of nursing practice.
Essential Functions

  • Participates in the pre-admission evaluation study and plan, as well as program design and placement of service recipients at the time of admission to GEC
  • Assesses, plans, implements and evaluates the development of individualized health care plans and/or emergency care plans for each service recipient with acute or chronic health concerns
  • Develops a written assessment for each service recipient in order to provide for nursing services as part of the total habilitation program. (Quarterly for ICF’s and semi-annually for IRA’s.)
  • Maintains ongoing progress notes for each service recipient
  • Periodically re-evaluates the type, extent and quality of services and programming for each service recipient. Modifies the written assessment in terms of the service recipient’s daily needs at least semi-annually by taking part in interdisciplinary team meetings
  • When appropriate, develops discharge plans and referrals to appropriate community resources. Maintains relationships with community programs in order to be able to provide knowledgeable referrals
  • Provides informal teaching moments on a one-on-one basis to service recipients. Participates in the planning and implementation of educating staff on pertinent health topics such as asthma, allergies, diabetes, child abuse, seizure disorders, eating disorder, infection control, first aid, HIPAA regulations, personal hygiene, OSHA, HIV/TB/Hepatitis, etc. Prepares written materials and presentations as appropriate
  • Ensures appropriate controls are in place to protect service recipients against communicable diseases and infections through identification and assessment of symptoms, reporting to appropriate medical authorities, and implementation of appropriate protective and preventive measures. Administers hepatitis vaccine and other immunizations when necessary
  • Weighs and takes vitals for each service recipient on a monthly basis ensuring the data is posted in the service recipient’s record. Reviews monthly nutritionist assessment (ICF’s) and reviews and signs off on pharmacy reviews (ICF’s quarterly and CR/IRA’s semi-annually) and follows up on recommendations
  • Meets with physician as needed to order and renew scripts – usually semi-annually for standard medications. Obtains consent from family/guardian/CAB and Human Rights for any behavioral medication provided by medical specialists. Provides updated medical information to families and day programs
  • Keeps abreast of all governmental regulations as they pertain to the nursing function in an OPWDD regulated residential setting and; maintains strict confidentiality regarding service recipient health information
  • Orders medications and medical supplies and reviews service recipients needs to ensure proper medication and supplies are readily available
  • Arranges medical appointments and accompanies a minimum of two (2) service recipient s each week on medical appointments. Visits service recipients in hospital within 24 hours of admittance and coordinates health care and discharge
  • Provides staff training to ensure they are able to

1) detect signs of illness or dysfunction that may warrant medical or nursing intervention
2) provide basic skills required to meet the health needs and problems of the service recipients; and
3) administer first aid in the event of an accident or illness

  • Trains staff on a regular annual basis in the areas of OSHA, oral hygiene, nutrition, hepatitis, measurement techniques re pulse, respiration and temperature. Conducts in-service in each specific medical area where actual service recipient involvement exists, i.e., seizure and seizure control, diabetes, etc. Provides AMAP certification and recertification when required
  • Develops written curriculums to cover the training needs outlined above; and maintains records of training session dates and will provide written evaluations of effectiveness of the course
  • Pinpoints the need for additional in-service training. If training cannot be provided by GEC staff, the nurse will find outside resources to provide the training
  • Provides on-call 24/7 cell phone availability for medical emergencies. Responds within 30 minutes of the call and provides telephone triage. Acts as back-up for nurses assigned to other residences as needed
  • Performs other related responsibilities as assigned

  • Knowledge of and ability to use a computer and work in a Microsoft windows software environment

Requirements:

  • Bachelors of Science or Associate’s degree in Nursing from an accredited college or university
  • At least one-two years as a practicing registered nurse in the delivery of community-based health services to the developmental disabled population
  • Valid Registered Nurse New York State License
