Position Summary for CNA
Provides basic home care skills and personal care activities following the Aide Assignment Sheet/Plan of Care written by the RN Supervisor.

Responsible for client care monitoring and communication with other appropriate team members and supervisor(s). Responsible for following agency policy regarding procedures or duties that an aide may perform.
Education/Qualifications
Currently listed as a Nurse Aide on the North Carolina Division of Health Service Regulation or proof of competency for aides not listed on the North Carolina Nurse Aide Registry.
Job Factors
Physical Requirements: Requires considerable physical effort most of the day. Requires squatting, kneeling, reaching, twisting, walking and sitting for long periods of time.

Mental Requirements:Work involves a broad range of problem solving, may be of a complex nature. Must be able to work independently, make sound judgments based on assessment and data available and act accordingly. Must be flexible, innovative, and possess good interpersonal skills. Must be able to prioritize multiple functions and/or tasks.

Hours: Must be able to work a flexible work schedule including weekdays, weekends, and evenings and holidays.
In-home aide duties
In-home Aide may include some of the following:

  • help with prescribed exercises
  • provide or assist with ADLs;
  • assist client with self-administration of medications.
  • perform IADLs
  • and record and report changes in the client's condition.
  • document tasks completed

Transportation
Must have valid North Carolina Driver's license and appropriate automobile liability insurance. Must have dependable transportation and ability to travel from assignment to assignment.
Responsibilities and Duties and Performance Standards Reports to work as scheduled and is regular in attendance.
Arrives in a timely manner without excessive absences and follows procedures for proper notification of absences/tardiness. Reports to work with a clean, well-groomed appearance.

Promotes a desirable work environment/attitude among fellow staff members. Conveys respect for staff and clientele. Maintains a positive attitude at all times.

  • Displays a degree of flexibility in work assignments.
  • Acts as a team member and assists other team members to meet work requirements.
  • Maintains a flexible schedule to meet the organizations work requirements. Acts as a preceptor to assist with new employee orientation.
  • Accepts client assignments and displays flexibility with schedule.
  • Review’s schedule weekly and accepts PRN assignments or duties as directed by scheduler, nurse or supervisor.
  • Follow the client’s plan of care/aide assignment sheet.
  • Performs/provides care or tasks specific to client needs as ordered on assignment. Demonstrates safe aide skills and competence.
  • Reports as needed to the supervisor and/or other designated resource person any inability to perform skills or incompetence for any procedure. Notifies nurse or appropriate designated personnel immediately as needed with any client problems or change in status.
  • Documents according to established agency procedure.
  • Documents daily the tasks performed, care provided, and any communication concerning client. Utilizes time sheet daily according to procedure.
  • Maintains a clean and safe working environment.
  • Follows all standards for universal precautions, infection control and agency policies when reporting to assignments.'
