Your Specific Responsibilities:


Tuberculosis

Provides TB case management including case consultation and oversight of TB program staff. Conducts contact investigations and coordination of legal processes for recalcitrant patients. Provides guidance to local TB program personnel, when appropriate.

Conducts home visits and hospital visits for treatment assessments of TB and Latent Tuberculosis Infection (LTBI) patients and families. Assists with Direct Observed Therapy (DOT) and Direct Observed Preventative Therapy (DOPT) when needed. Inputs all information into the appropriate electronic data system no later than the next business day.

Ensures compliance with State standards (activity reporting, supply accountability, and patient confidentiality). Reviews laboratory and radiology reports and other medical data pertinent to diagnosis and treatment of TB and LTBI. Advises DOH-Volusia and TB Network physicians concerning this data. Dispenses appropriate medications based on consultation with TB physicians or per nursing protocols.

Implements program specific assessments. Assists in the completion and aggregation of required TB program management reports: compiles statistics, analyzes data, and completes reports.

Reviews county specific TB metrics at least monthly. Implements a robust quality assurance/improvement program to improve delivery of services.

Provides TB program guidance to ensure effective communication and outreach to community health care providers and impacted populations. This includes outreach projects which target appropriate community-based organizations, correctional facilities, and health care institutions in screening and follow-up for high risk groups.

Conducts at least one significant outreach activity every two months.

Conducts TB testing using IGRA blood tests or PPD for new personnel and others as required.

Monitors HMS at least weekly for positive IGRA and/or PPD tests. Evaluates and treats patients for LTBI as indicated. Monitors Electronic Disease Notification (EDN) system at least weekly and initiates case evaluation within 30 days.

Completes and updates the TB infection control plan (including respiratory protection) annually.

Compiles and submits written articles to the Epi Log Newsletter for publication at least annually.


Sexually Transmitted Diseases (STD)

Provides sexually transmitted disease (STD) case management including case consultation, testing, and treatment as required for STDs including syphilis, gonorrhea and chlamydia.

Provides guidance to clinical personnel and disease intervention specialists (DIS) concerning STDs. Provides education and counseling to STD patients and their partners as required.

Assists with contact trancing and treatment of contacts as appropriate.


Epidemiology

Assists Epidemiology program as necessary with communicable disease reports, outbreaks, and/or prevention activities.

Conducts case interviews as required.

Collects clinical samples for shipment to laboratory.

Provides immunizations, rabies prophylaxis, and other prophylaxis/preventative treatment as needed.


Human Immunodeficiency Virus (HIV)

Works within the scope of practice providing services to HIV program patients as needed in CHD clinics.

Provides counseling and/or sample collection for HIV and viral hepatitis testing

Provides nursing consultation for HIV case managers and others as required.


Required Knowledge, Skills, and Abilities:


Knowledge of nursing principles, practices and techniques in specialized areas

Knowledge of TB, HIV and STD prevention and treatment guidelines

Skilled at disease investigations


Qualifications:


Minimum -

Must have knowledge of nursing principles, practices and techniques in specialized areas

Must have knowledge of TB, HIV and STD prevention and treatment guidelines

Must be skilled at disease investigations

Must be currently licensed as a Registered Nurse in the State of Florida and have one year of professional nursing experience or a Bachelor's degree from an accredited college or University with a major in nursing and licensure as a Registered Professional Nurse in accordance with Florida Statute 464 or eligible to practice nursing in accordance with Florida Administrative Code 210-8.27.?

Must have experience as a Registered Nurse in performing a nursing assessment, providing education and/or providing any immunizations?

Must have experience counseling teens, young adults, adults on immunizations, HIV, family planning or sexually transmitted diseases?

Must have experience working in a clinic or community setting and able to multi-task and work efficiently and accurately under pressure?


Preferred -

Knowledge of DOH disease control computer programs (i.e. Merlin, STARS, HMS, FLSHOTS, etc.)

Ability to communicate effectively both verbally and written
