Your Specific Responsibilities:
This is an advanced specialized professional position at the Florida Department of Health in Monroe County. The incumbent will be the department’s “Community Outreach Coordinator” and be required to provide technical support along with staff and help to improve health outcomes, advance health equity (HE), and overcome COVID-19 health disparities (HD) within the county. The incumbent will be responsible for overseeing three projects: HE, HD, and Healthy Communities.
This is a full-time position that requires some weekend and evening hours, travel and emergency duty as assigned. This position reports directly to the Public Health Services Manager F - SES.
Minority Health and Health Equity:
Facilitates the continuous development of the health equity plan in Monroe County according to the standards of Florida’s Office of Minority Health (OMH). This position leads and guides health equity task force partners in planning and implementing community projects addressing the social determinants of health affecting the health disparity for Monroe’s identified priority populations. Leads and guides a health equity coalition in overseeing and providing feedback for health equity plan activities. Leads and guides a health equity team of internal DOH stakeholders in advancing issues of health equity within the department.
- Updates health equity plan quarterly for publication and widespread distribution throughout the county by:
- Identifying or collecting new local data sources to analyze health and social determinants of health (SDOH) disparities at the county or census level, and interpreting this data for task force consideration in project development
- Facilitating the development of new projects to address community needs as identified by disparities in mental health and SDOH data for priority populations
- Ensures the success of all projects included in Monroe’s health equity plan by:
- Assisting in implementing project activities as needed
- Securing grant funding to support existing projects and new initiatives of the task force
- Working with task force project leads to obtain project objective data for tracking and reporting
- Provides regular written updates to Florida’s OMHHE, including:
- Quarterly reporting on health equity project objectives and activities via ClearPoint Strategy software
- Quarterly reporting on health equity infrastructure, partnerships, activities, events, and personnel/resources via ClearPoint Strategy software
- Biweekly reporting on status of all related activities via word document to OMHHE Regional Coordinator
- Participates in Florida OMH, Monroe DOH meetings, and external community meetings to advance health equity
- Manages budget in partnership with program manager.
- Researches and writes grants as needed to facilitate the department’s health equity needs, in addition to grant-related duties to support health equity plan projects.
- Participate in state trainings, including the Health Equity Training Program.
Health Disparities
- Continuously serves as the Health Disparities Liaison and assures that all activities and deliverables are met according to the statement of work.
- Build, leverage and expand infrastructure support for COVID-19 prevention and control among populations that are at higher risk and underserved
- Establish local health equity councils/coalitions and develop health equity plans.
- Engage local partners in development, execution, and sustainability of Community Health Improvement Plan
- To strengthen the collaboration with health care providers by promoting and maintaining health; and preventing and managing disease
- Work with various agencies and organizations in developing and disseminating comprehensive health provider directory. Directory will showcase services that offer telehealth and serve: uninsured, underinsured, Medicare, and Medicaid patients.
- Improve community awareness and utilization of mental health and substance use services
- Institute policy, systems and environmental structures to address social determinants of health and mitigate disproportionate burden from emergencies on vulnerable communities.
- Engage stakeholders to consider health in all policy decisions and government operations
- Engage community members in setting health equity priorities through use of PACE-EH methodology
Healthy Communities
- Continuously serves as the Healthy Communities Liaison and assures that all activities and deliverables are met according to the Workplan.
- Complete required strategies in the following priority areas: (1) Nutrition and Physical Activity in Early Care and Education Settings, (2) Nutrition and Physical Activity in School Settings, (3) Nutrition and Physical Activity in Community Settings, (4) Worksite Wellness in Community Settings, (5) Chronic Disease Prevention – Resource Linking Awareness, and (5) Integration of Chronic Disease Prevention Strategies in the Community Health Improvement Plan.
- Participate in all conference calls.
- Submit quarterly reports and success stories on time as required.
Community Health
- Participate and represent FDOH Monroe at community events and provide an overview of the department’s programs.
- Provide support as necessary to the Public Information Officer to ensure communications are culturally and linguistically competent.
- Procures items and media needed to manage the needed outcomes, that are culturally and linguistically competent and receives invoices in My Florida Marketplace to achieve the purchase.
- Participates in quality improvement and activities to improve processes and systems in place.
Performs other related duties necessary to meet the goals and objectives of the Florida Department of Health in Monroe.

Qualifications:
Minimum
- Maintains current Florida Driver’s License
- Bachelor’s degree from a college or university and two years of professional experience in public health, health sciences, program planning, or an agency area.
Preferred
- Master’s degree from a college or university in public health, health sciences, program planning, or an agency area. Professional or non-professional experience as described above can be substituted on a year for year basis for the required college education

Emergency duty required of the incumbent includes working in special needs or Red Cross shelters, or performing other emergency duties including, but not limited to, responses to or threats involving any disaster or threat of disaster, man-made or natural.
