Essential Functions

  • Displays Knowledge of and compliance with all applicable federal and state hospice rules and regulations.
  • Participates in recruiting, interviewing, and recommending of employment of qualified staff.
  • Coordinates and is responsible for new employee orientation and ongoing education for all staff.
  • Monitors and assists with control of overtime, travel, durable medical equipment (DME) utilization and medication costs.
  • Completes monthly staff schedules to ensure adequate staff to meet patient and family care needs 24 hours a day, 7 days a week, every week of the year.
  • Serves on committees as assigned.
  • Supports QAPI activities through staff education, result monitoring and performance improvement initiatives.
  • Fosters continuous improvement and innovation by identifying and implementing improvements that increase efficiency and enhance work quality.
  • Communicates with staff on regular basis. At direction of Clinical Services Manager, schedules and leads staff meetings.
  • Delivers clear messages, endures understanding, listens, and responds effectively. Prepares clear, concise and well organized written documents and oral presentations. Facilitates open communication.
  • Ensures that all staff adheres to all written Policies and Standards of Performance in compliance with all applicable federal, state, and local laws and regulations.
  • Ensures that care and services from staff are provided in a manner consistent with accepted standards of practice and licensure. Ensures that care optimizes the comfort and dignity of the patient and is consistent with the patient/family needs and goals. Utilizes Interdisciplinary Team (IDT) to meet those goals.
  • Ongoing review of chart documentation and MARS to ensure quality of care is maintained and regulations are met.
  • Collaborates with all departments to assure clinical staff cooperation and support to accomplish the organization’s goals.
  • Proactively anticipates, manages, and constructively resolves conflicts and disagreements.
  • Monitors CAHPS Survey results to determine family satisfaction of care.
  • Builds and maintains relationships with external customers/referral sources to share information, and establish partnerships to maximize the organization’s work of hospice. Serves as advocate of patients, families, external customers, the staff, and self

Trains and develops employees, provides constructive performance feedback and appraisals, and takes appropriate corrective action to address performance and conduct issues in an environment of respect, compassion, and trust. Models fairness and conveys respect.
  • Maintains clinical skills and expertise in end-of-life symptom management. Serves as resource to staff.
  • Coordinates and directs IDT meetings. Ensures compliance with related Medicare guidelines.
  • Collaborates with hospice medical director, associate medical directors and nurse practitioners in the management of overall plan of patient care.
  • Participates in weekend, holiday, and administrative on-call. Documentation is timely if required to complete a patient visit.
  • Supports, demonstrates, and celebrates evidence of Hospice of Southern Illinois core values.
  • Appropriately manages Paid Time Off.

Patient Care Supervisor Position Summary : Management and supervision of licensed and certified clinical staff. Responsible for the management of day-to-day clinical operations of the team and coordinates all aspects of patient care. Provides employee coaching that facilitates development of the person and delivery of the highest quality of patient/interdisciplinary care. Assures consistent, appropriate, and cost-effective care by actively managing patient/family’s needs, payor expectations, company policy compliance, and Hospice of Southern Illinois’ financial objectives. Professionally communicates with staff, patient/families, referral sources, and community members. Provides operational and administrative support to the clinical services manager.

  • Full-Time Salaried Exempt Position
  • 40 Hours Weekly, Monday-Friday 8:30 A.M. - 5:00 P.M. with a rotational administrative on-call shift. The administrative on-call shift provides support to the clinical staff after hours. Typically, it is only telephonic support only unless there is an emergency. The administrative on-call shift averages to every other week starting on a Friday 5:00 P.M. and ending the following Friday at 8:30 A.M.

Position Requirements
a. Education
1.) Graduate from a State-approved school of nursing.
2.) BSN preferred.
b. Qualifications
1.) Two (2) years hospice experience required.
2.) Two (2) years’ experience as a hospice admission nurse is preferred.
3.) One (1) year management/leadership required.
c. Other Qualifications
1.) Current Illinois nursing license. (RN Required)
2.) Certification in hospice and palliative care preferred.
3.) CPR certification preferred.
4.) Excellent leadership, managerial ability, and interpersonal relationship skills.
5.) A working knowledge of Medicare/Medicaid, hospice regulations, and third-party payor expectations is preferred.
6.) Reliable means of transportation.

Physical Requirements: Performs repetitive tasks. Does sedentary work: prolonged periods of sitting, and exerts up to 10 lbs. force occasionally. Requires physical flexibility of bending at knee and squat-bend. The Patient Care Supervisor manages stress appropriately, makes decisions under pressure, handles multiple priorities, works alone, and manages anger/fear/hostility/violence of others appropriately. This position requires clarity of vision >20 inches and <20 feet, hears alarms and telephones.

Working Conditions: The Patient Care Supervisor spends approximately 95% of their time in an air-conditioned environment with varying exposure to excessive humidity and noise. The remaining 5% of his/her time is spent in an automobile or patient home, which may or may not have heat/air conditioning. The position has potential for exposure to malodorous, infectious body fluids from patients, animal pet hair, allergens in the home environment, and some exposure to noxious smells from cleansing agents, exposure to communicable diseases, and traffic.
