We are seeking a compassionate and dedicated Personal Care Assistant to work with individuals with developmental and/or physical disabilities in a community-based service setting in Minnesota. The PCA will provide assistance and support to help individuals live independently and achieve their goals. The PCA will report to the Program Manager.

Key Responsibilities:

• Assist individuals with activities of daily living (ADLs) such as personal hygiene, grooming, dressing, and meal preparation.
• Support individuals with medication management and administration.
• Assist individuals with scheduling and attending appointments, social events, and other activities.
• Provide companionship and emotional support to individuals.
• Assist individuals with behavioral challenges in developing coping strategies and positive behavior choices.
• Provide transportation to and from appointments, activities, and events.
• Maintain accurate and timely documentation of services provided in electronic health records (EHR) system.
• Ensure the safety and well-being of the individuals.

Qualifications:

• High school diploma or equivalent required.
• Must have a valid driver’s license and reliable transportation.
• Completed training program approved by the Minnesota Department of Human Services (DHS) for Direct Support Professional or have completed the competency evaluation from the DHS.
• Ability to communicate effectively in English, both verbally and in writing.
• Ability to read and interpret written instructions and document client care accurately and timely.
• Ability to follow a care plan and work independently with minimal supervision.
• Ability to lift and transfer clients who may require assistance, up to 50 pounds.
• Current CPR and First Aid certification preferred.
