We are seeking a full time Peer Adherence Educator to support our Queens site.

SUMMARY OF POSITION:

Responsible for educating patients and the community regarding disease state, prevention, risk reduction, benefits of treatment, and the importance of medical and medication adherence; support patients by sharing effective strategies to manage their health; share lived experience to engage and support patients. Demonstrates cultural competency in working with diverse populations using an anti-racist framework and equity lens.

ESSENTIAL FUNCTIONS:

Escorts patients to appointments and/or lab services as needed.

Works with members of the multidisciplinary care team to help improve patient health outcomes.

Participates in Peer Certification Training and continuing education as required.

Provides community outreach and community education programs to targeted community agencies.

Provides appropriate follow-up on referred patients.

Participates in case conferences as well as other agency or community-based meetings or trainings to promote program and fosters linkages with external community partners.

Implements evidence-based strategies

Works with patient and care team to support patient's retention in care.

Maintains progress notes and case records as required in EMR and relevant data programs

Provided harm education and adherence education.

Provides referrals to those in need of support services and/or other community resources.

Provides linkage, support and education to newly diagnosed or new to care patients within Sun River.

EDUCATION/EXPERIENCE:

High School Diploma/GED with 1-2 years relevant experience
