We are seeking a full-time
Home Health Physical Therapist to join our healthcare team in
Lynwood, WA. The Physical Therapist will provide skilled physical therapy services to patients in their homes, including evaluations, treatments, and patient education. The PT will work collaboratively with the healthcare team to ensure the patient's needs are met and documented accurately.
Responsibilities:
- Conduct comprehensive evaluations of patients' physical abilities and develop individualized care plans in collaboration with the healthcare team and the patient's family.
- Develop and implement treatment plans to improve patients' mobility, strength, and function.
- Educate patients and their families on exercises and techniques to promote healing and prevent injury.
- Document all patient care and observations in the electronic medical record.
- Monitor and report changes in patient's physical abilities and progress to the physician and healthcare team.
- Collaborate with other healthcare professionals to ensure continuity of care for patients.
- Ensure compliance with regulatory and accreditation standards.
- Participate in ongoing education and training to maintain and enhance clinical knowledge and skills.
Requirements:
- Active Physical Therapist license in the state of Washington.
- Minimum of 1 year of experience in home health or a related field.
- Excellent communication and interpersonal skills.
- Strong clinical assessment and critical thinking skills.
- Ability to work independently and collaboratively with the healthcare team.
- Valid driver's license and reliable transportation.
- Basic computer skills and familiarity with electronic medical records.
