Position Summary:

The Per Visit Physical Therapist functions as a key member of the Homecare department through the administration of Physical Therapy services under physician orders in accordance with CMS and DPH Home Health regulations, best practice and quality standards.

• 
  Advanced Total Joint REplacement Program
• Evidence Based Fall Prevention Programs (Tai Chi/Otago/CT Falls Collaborative)
• Lymphedema Therapy
• PHIT (Pelvic Health and Incontinence Program)
• Modalities (Kinesio Taping/Biofeedback/Ultrasound/TENS)
• CORE: Oncology Rehabilitation
• Rehabilitation: Pulmonary, Cardiac and Vestibular Rehabilitation (including Epley Maneuver/Canalith Repositioning)
• Geriatric Certification
• Pediatric Rehabilitation
• Hospice/Palliative PT/OT/ST
• Cognitive Testing (“MOCA” Montreal Cognitive Assessment)
• Orthotic/Prosthetic Evaluation, Fitting and Training
• Low Vision Assessments and Home Modifications
• Maintenance Care (ALS/MS)

Minimum Qualifications:

• 
  Graduate of an accredited School of Physical Therapy
• Licensed or eligible for licensure to practice physical therapy in the State of Connecticut

Preferred Qualifications:

• 
  Experience with electronic medical charting
• 3-5 years’ clinical experience
• Flexibility with scheduling
