As a Physical Therapist (PT) for PRN Home Health & Therapy you will provide high quality and compassionate care to patients in Fond du Lac, Waupun or Oshkosh. This physical therapist position would work in either the skilled nursing facility setting, home health or both (if full-time).

This position is great for a Physical Therapist who is compassionate, a great communicator, likes working independently and as part of a team, and wants to either pickup additional hours this week or change roles!

Skills, Knowledge, and Abilities: Adaptability, dependable, detail-oriented, compassionate, self-starter. Experience preferred, but new graduates are welcome to apply.

Job Requirements

· Respond to and initiate patient referrals

· Review available patient information and evaluate the impact of this information on the assessment and treatment process

· Evaluate and treat patients in accordance with orders from the physician through the use of therapeutic procedures designed to address deficits in physical function and improve independence, safety and quality of life.

· Comply with evaluation, treatment and documentation guidelines.

· Conducts and documents a thorough functional assessment of the patient utilizing standardized procedures

· Record patient progress in the patient record a minimum of every ten (10) days.

· Documents the course of treatment in a clear, concise manner, including a discharge summary utilizing the prescribed format.

· Instruct Assistants in treatment procedures to be utilized and provide supervision while such staff is working with patients.

· Participate in the quality improvement process by responding appropriately to results of medical record audits, patient/referral sources, satisfaction surveys, safety activities, and regulatory requirements.

· Assess Physical Therapy Assistants’ competency on an annual basis per individual.

· Act in compliance with company policies and procedures, State, Federal and Professional Organization regulatory and professional standards and guidelines.

· Promote the programs and services of PRN Home Health and Therapy through formal and informal interactions with the community.

· This position is considered nonexempt under the Fair Labor Standards Act.

Education Requirements:

· Graduate of an accredited Physical Therapy curriculum.

· Current license/certification in Wisconsin.

· Current American Heart Association CPR Certification.
