Responsibilities:
Basic Function/Purpose: Facilitate the transport of individuals and families to shelter. Conduct initial
diversion/intake screening prior to contacting 211 to verify homelessness eligibility of individuals and
families. Support the quarantine placement of COVID individuals and families in DSS-contracted hotel.

Essential Functions/Responsibilities:

1. Respond to all incoming calls from clients and/or providers
2. Complete a diversion screening with walk-in clients prior to calling 211 to confirm homelessness
eligibility
3. Pick up clients and transport them to the appropriate shelter as determined by 211/DSS
4. Support quarantine of COVID clients through proper documentation prior to their hotel
placement, transportation to quarantine, and providing food, hygiene items, hotel rules
5. Work with designated hotel agencies/shelters to provide a warm client hand off
6. Maintain and update transportation logs and documentation via shift notes
7. Communicate with other shift Program Associates/Caseworkers/admin as needed
8. Ensure there is adequate staffing prior to leaving a shift
9. Observes all safety rules and regulations including use of personal protective equipment
Qualifications:
Education and/or Experience Required at Entry: H.S. Diploma or G.E.D. with up to three (3) years relevant experience or an equivalent combination of education and relevant experience. Experience working with homeless population preferred.

Working Conditions and Environment/Physical Demands:
Mobility sufficient to drive to and participate in meetings at client’s homes and/or community facilities which may not meet Federal accessibility standards for disabled individuals. Mobility sufficient to work in an office environment. Manual dexterity sufficient to operate office equipment including machines.


Licenses/Certificate/Clearances:
Valid N.Y.S. Driver’s License, N.Y.S. Central Registry clearance.


Required Trainings:
VIRTUS, Corporate Compliance, Core Updates.
