Psychiatric Rehabilitation Program Counselor Job at Park West Health Systems, Inc
The Psychiatric Rehabilitation Program Counselor is responsible for the direct provision of rehabilitation and support services to participating consumers. The PRP Counselor visits consumers in their homes, schools and communities, and provide services in the home, community, schools or program site.
ESSENTIAL DUTIES AND RESPONSIBILITIES INCLUDE THE FOLLOWING
Other duties may be assigned as necessary
- Conduct regularly scheduled consumer visits.
- Provide identified PRP rehabilitation and support services in accordance with the consumer's Individual Rehabilitation Plan (IRP).
- Conduct Rehabilitation groups and activities to address rehabilitation and support service needs.
- Maintain required and consistent documentation for each client, including contact/progress notes, monthly summaries, and quarterly reviews.
- Participate in ongoing trainings and supervisions.
- Shares/transmits the mission and vision of Arrow Child & Family Ministries to staff and to clients and families served.
- Maintain a manageable caseload to ensure outcomes and practice requirements are met while taking into consideration the level of care of clients served.
- Must have own transportation
- Perform all duties within contract, state and federal regulations, Arrow policy, and COA standards.
Education, Licensure and/or Experience
An Associate's Degree or equivalent college credits from a four-year college, in social work, psychology, mental health or another related human services field. At least one (1) year of previous experience working with youth in a mental health setting.

Pay: $52,000.00 per year
