Job description
Berkley East Healthcare Center is a beautiful facility located in Santa Monica that provides skilled nursing care to our clients. We are currently looking for Registered Nurse's (RN) to join our team on the AM, PM or Night Shifts.
Our RN’s are critical in providing excellent patient care. They help with passing medication, identifying changes of condition, IV’s, TPN enteral feeding, wound care, and much more.
We are looking for experienced and new grads RNs to support medically complex seniors in an effort to better manage their health.
Day to day:
- Nurse care management: assess, plan, implement, coordinate, monitor, and evaluate the plan of care with the attending physicians.
- Provide well-documented, accurate and sensitive in-person advice, health education and self-care instruction.
- Support patients during times of crisis or transition to prevent avoidable hospital readmissions.
- Reach out to patients identified as high-risk.
Qualifications:
- Valid CA RN license.
- Ability to thrive in a fast-paced environment and adapt to change.
- Ability to work both independently and collaboratively within a team.
- Demonstrated clinical assessment skills.
