This Registered Nurse (RN) Case Manager position will provide care for patients throughout Franklin County. Must have a current Florida RN license and a minimum of 3 years RN experience preferably in Hospice, Home Healthcare, or Nursing Homes. Must also have a valid driver's license.

Your Impact
As the RN Case Manager, you will perform the initial assessment and participate in the comprehensive assessment and ongoing updates to assess the impact of the terminal diagnosis on the patient’s physical, functional, psychological, and environmental needs and activities of daily living.
Your Main Duties and Responsibilities
(Includes but, not limited to)
- Visit patients where they consider home to be whether it is their residence or a facility.
- Provide education on medication, disease prognosis, and the end-stage disease process.
- Coordinate care and medications with MD and APRN and utilize a team approach.
- Utilize an interdisciplinary team approach to caring for the patient and their family.
- Provide routine wound care, recommendations, and assessments.
- Demonstrate knowledge of drains, trach, and PEG tube care and maintenance.
- Manage and implement the patient’s care plan according to state regulations and communicate changes to physicians and team members involved in the patient’s care.
- Assess, monitor, document, and report the patient’s health and condition using required documents, reports, and required timelines.
- Use an Electronic Medical Records (EMR) System for charting.
- Ability and willingness to travel throughout the Franklin County Service area (Apalachicola, Carrabelle, Eastpoint, Alligator Point)
Schedule: 7:30 am to 4:30 pm Monday – Friday (1 holiday per year on call requirement)
Required Skills and Education:
- Must have an active Florida RN license in good standing
- BSN is preferred
- Minimum of 2 years of nursing experience required
- Hospice experience is a plus
- EMR experience required
- Strong computer skills are required with a working knowledge of Microsoft Outlook and Team
