RN Multi-site Case Manager

Registered Nurse Required

CommuniCare Health Services is currently seeking a Registered Nurse with case management experience for a full time RN Multi-site Case Manager position to oversee facilities in the Cincinnati, OH area. The RN Multi-site Case Manager is mostly a remote position, with travel required to the individual facilities as needed. Salary commensurate with credentials and experience.

The RN Multi-site Case Manager job duties include:

• Case management
• Precerts, authorizations, and concurrent reviews
• Management of utilization, therapy minutes, insurance requirements, etc
• Coordination and communication with insurance companies.
• Working with the IDT team

POSITION REQUIREMENTS

Qualifications

• Graduate of an accredited School of Nursing
• Valid RN license in the state of Ohio
• Experience working in Long Term Care
• Experience working with Managed Care Services, preferably in a Long Term Care environment
• Experience with the case management process

Knowledge/Skills/Abilities

• Customer service focused
• Professional and polished over the phone
