Job Description

We are seeking a dynamic and experienced Registered Nurse (RN) to join our team as a Health and Wellness Coordinator. In this role, you will be responsible for overseeing the health and wellness of our clients, employees, and community members. You will work collaboratively with interdisciplinary teams to develop and implement wellness programs, provide health education, and coordinate health care services. Responsibilities: Develop and implement health and wellness programs for clients, employees, and community members, using evidence-based practices and innovative approaches. Provide health education and counseling to individuals and groups on topics such as nutrition, exercise, stress management, and disease prevention. Coordinate health care services, including primary care, specialty care, and mental health services, for clients in collaboration with interdisciplinary teams. Develop and implement infection prevention and control protocols, and ensure compliance with regulatory requirements and best practices. Manage health and safety issues, including emergency response, first aid, and medication administration. Collaborate with interdisciplinary teams to develop care plans and coordinate care for clients with complex health needs. Monitor and evaluate health and wellness programs and services, and adjust as needed to ensure optimal outcomes. Maintain accurate and complete documentation of all assessments, interventions, and progress notes in accordance with regulatory requirements and best practices. Participate in team meetings, case conferences, and quality improvement initiatives to ensure effective and efficient care delivery. Stay up-to-date with the latest research and best practices in nursing and health care, and incorporate new approaches and technologies into programs and services as appropriate. Qualifications: Bachelor's or Master's degree in Nursing from an accredited institution. Current state licensure and registration as a Registered Nurse. Strong leadership, communication, and teamwork skills. Ability to work independently and as part of a team. Passion for promoting health and wellness, and improving the quality of life for individuals and communities. Strong problem-solving skills, and ability to develop and implement creative solutions to complex health care issues.
