Primary Function/General Purpose of Position
RETAIN is a grant funded joint initiative between Mercy Health, Ohio Department of Jobs and Family Services, the Department of Labor, and the Social Security Administration. RETAIN is a Stay at Work/Return to Work program (Retaining Employment and Talent After Injury/Illness Network). The RETAIN Nurse Supervisor provides services to injured workers by coordinating return-to-work activities with all interested parties and assisting medical providers, injured workers and employers toward a safe and timely return to work plan, ultimately reducing unnecessary work disability time. Manages Health Services Coordinator (HSC) operations by initiating, coordinating, and improving upon current policies and procedures. Actively participates in marketing and carrying out activities with RETAIN Program Manager to these goals, responds to participants, physicians, and team members in a manner that reflects professionalism and that demonstrates BSMH Mission and Values. This is a grant funded initiative with a plan to extend beyond grant duration based on program growth and success.
Essential Job Functions
Coordinates the full cycle of the injured patients’ case from onset to completion. Advocates for all parties and serves as a liaison on behalf of the provider to ensure the injured patients’ get back to work in some capacity as soon as safely possible. Tracks, addresses, and communicates barriers that prevent a patient from returning to work and develops strategies to address the issues. Ensures care coordination among providers, staff, and employers to improve clinical outcomes.
Assists providers in completion of return to work documentation. Accesses and reviews provider documentation to establish patients’ severity level, review providers’ plan of care, and address any barriers to plan of care.
Guides HSC team members to maintain a compassionate, healthy, collaborative team environment. Ensures the patients’ plans of care are developed, implemented, and evaluated. Actively pursues proper utilization, discharge planning, progress toward goals, and problem solving.
Adopts and implements innovative nursing practices to improve RETAIN program. Receives and reviews standard reports with other RETAIN team members to continue to improve the program’s efficiencies and promote success. Collects and organizes data that monitors the effectiveness of the RETAIN health care delivery system.
Collaborates with RETAIN Program Manager to determine a staffing pattern which will accomplish stated objectives and promote maximum level of utilization of personnel. Participates in performance improvement activities and contributes to program development to facilitate accomplishment of departmental goals. Holds team accountable to productivity standards set forth. Participates in developing objectives for the agency. Displays a willingness to support the policies and procedures and follows appropriate channels for change. Leads regularly scheduled team meetings, in-services, case conferences and maintains documentation.
Responsible for recruiting, hiring, evaluating, engaging, retaining and terminating serve personnel. Participates in planning for orientation of new employees and education of existing employees. Conducts selected orientation/education classes as needed.
Responsible for completion of accurate and constructive performance evaluations in order to develop a high-quality team. Proactively monitors team and individual performance. Provides coaching, mentoring and action plans for team members.
Expands knowledge base through self-development. Responsible for Human Resources functions and corrective action processes. Continually updates own knowledge and serves as information resources for staff.
