Responsibilities:
Provide social work services to residents, their families, and significant others, consulting and interacting with other disciplines as a team to achieve individualized, comprehensive, high-quality care. Facilitate the resident's admission in the facility and perform resident/family assessment. Provide initial and on-going therapeutic intervention and social work services to the resident. Adhere to and document activities in accordance to with Social Work policies and procedures, NYSDOH and Joint Commission regulatory standards.
Qualifications:
- Master's in Social Work, Certified Social Worker or a Bachelor's Degree with relevant experience.
- Prior experience/exposure in health care setting required.
- Excellent organization and oral and written communication skills.
- Must be able to read, write and speak the English language.
