Positions Responsibilities:
The Social Worker in this position will serve as a Care Manager for Albemarle Regional Health Services, serving in the Care Management for At Risk Children (CMARC) and Care Management for High Risk Pregnancy (CMHRP) programs. The Care Manager is responsible for recruiting and assisting patients in accessing appropriate health and social care systems in both Prenatal and Child Health programs, with local medical offices, as well as through outside referrals. This position will be based within one of the Northeastern NC Partnership for Public Health counties. The counties covered under this partnership include: Beaufort, Bertie, Camden, Chowan, Currituck, Dare, Edgecombe, Gates, Halifax, Hertford, Hyde, Martin, Northampton, Pasquotank, Perquimans, Tyrrell, and Washington.
Key Duties:
- Provide care coordination services for clients and patients focusing on the organization of services and resources to respond to the needs of patients and their families. These services may include health care, special therapies, childcare, and counseling. Ultimately, service coordination enables families to assume coordination responsibilities with the support of a professional team.
- Virtual Health will be used for data entry.
- Assists clients in accessing the health care and social services systems in order that outcomes which contribute to physical and emotional health and wellness can be obtained.
- Care Management integrates and provides continuity of care with other services received.
- Ensure the provision of comprehensive preventive health services to eligible clients.
- Assist clients to meet other priority needs that affect their well-being and that of their families.
Knowledge, Skills, and Abilities:
- Considerable knowledge of social work principles, techniques, and practices and their application to individual casework group, work and community problems.
- Knowledge of the psychosocial, socioeconomic and behavioral problems and their treatment.
- Knowledge of governmental and private organizations and community resources.
- Knowledge of laws, regulations and policies which govern the program.
- General knowledge of medical terminology, disease processes and their treatment is required in certain programs or settings.
- Skill in establishing rapport with a client and applying techniques of assessing psychosocial, behavioral, and psychological aspects of client's problem.
- Ability to establish and maintain effective working relationships with administrative superiors’ members of case load and their families, and with civics medical, social and religious organizations.
- Ability to train or instruct lower-level social workers, students, or interns.
- Ability to express ideas clearly and concisely and to plan and execute work.
Minimum Training and Experience:
Bachelor's degree in social work from an appropriately accredited institution; bachelor's degree in a human services field from an appropriately accredited institution and one year of directly related experience; bachelor's degree from an appropriately accredited institution and two years directly related experience; or an equivalent combination of education and experience.
This position does REQUIRE one of the following degrees: Bachelor of Science in Social Work, Bachelor of the Arts in Social Work, or Bachelor Degree in Social Work.

Pay: $41,976.00 - $52,344.00 per year
