Evolve Therapy Services is a therapy management organization seeking energetic and passionate Speech Language Pathologists looking for stability with an In-house Skilled Nursing Facility therapy program for Champion Care Healthcare located in Oconto, WI
Job Position: Speech Language Pathologist
Job Type: PRN
Job Purpose:
To assist in planning, organizing, developing, and directing Speech Language Pathology services in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be direction by Administrator, to assure the highest degree of quality resident care is always maintained.
Job Functions could include:
- To provide quality speech therapy services and initiate referrals when appropriate, in accordance with established standards of practice, therapy department procedures and productivity standards.
- To evaluate and obtain data necessary for treatment planning and implementation
- To develop a treatment plan that includes a target list, short-term, and long-term goals
- Instructs patient, family, and staff in appropriate speech-language procedures to allow carry over of therapeutic remediation and goals in daily activities
- To complete compliant and medically necessary documentation from CMS expectations in NetHealth EMR systems
Education & Qualifications:
- Master's degree in Speech Language Pathology
- Certificate of Clinical Competency
- Current State license (or is license-eligible) in state of practice
- Ongoing continuing education and professional development to maintain current licensure and certification - Evolve Therapy Services provides unlimited CEU package through MedBridge partnership
