Registered Nurse Duties and Responsibilities:

• Conduct PRIME and TCNA assessments in the patient’s home or in alternate plan approved settings
• Assessments are face to face visits ONLY
• Non-touch assessment containing basic questions on IADL Mobility, Nutrition, and Environment
• Identify all active and chronic disease conditions as well as determine all physical, mental, and social needs present at the time of the visit
• Educate and counsel patient and family on any conditions identified during assessment

Registered Nurse Candidate Requirements:

• Must have clear and active RN, LVN or LPN South Carolina state or compact state license
• 2 years+ experience working with geriatric population required
• 2-3 years of case and/or utilization management required
• Ability to complete and clear a background and credentialing screen
• Must have valid state driver license and automobile insurance
• Strong computer skills: including but not limited to Word, Excel, EMRs and Web based applications
• Strong Attention to Detail
• Must be able to complete all HRAs and corrections (if needed) in a timely manner

Job Type: Part-time

Pay: $75.00 - $375.00 per day

Weekly schedule:

• Monday to Friday
• Weekend availability
