Position Purpose:
Serves as a contributing member of a treatment team. Ensures the application of the principles of developmental service to empower and assist consumers in the pursuit of heir rehabilitation goals and community integration. Ensures the coordination of comprehensive services to a designated caseload of consumers. Participates in the day to day program operations, performing supervisory and administrative leadership for staff. Assures for the timely and appropriate coordination of program treatment plans
Principal Responsibilities and Duties:
- Supervision of service coordinator and their caseload.
- Works collaboratively with the DS Funding Manager on funding needs, QA coordination, and clinical and med. Management
- Maintain a scheduled presence in assigned offices.
- Maintains alliances with family members of consumers.
- Facilitates and schedules meetings, assignments and activities as needed.
- Coordinates and monitors the development of Individual Service Agreement plans with Quality Assurance coordinator and Service Coordinator, for assigned consumers, which include strategies, interventions, time frames, identification of responsible individuals, locus of interventions and linkages with other providers, family and significant others necessary for successful attainment of identified goals. Assures consumer involvement in the planning process.
- Serves a primary case manager for consumers assigned to caseload.
- Ensures collaboration with members of the HCRS programs, teams regarding individuals on caseload and community resources.
- Oversight of case records and statistical data required for management information, billing and program evaluation.
- Periodically audits case records to ensure appropriateness and completeness of documentation.
- Coordinates, organizes and monitors data collection and entry regarding performance indicators, outcome measures and consumer satisfaction studies.
- Monitors and assigns caseloads to service coordinators and assures for the coordination of all direct consumer supports.
- Continually assess program needs, identifies and implements best practices.
- Coordinates and monitors the implementation of Utilization Review practices and procedures.
- Collaborates with other team members in the provision of properly scheduled, effective performance evaluations based on employee job descriptions.
- Communicates necessary information from senior leadership to staff.
- Develops and implements programs of educational value to communities as part of HCRS community involvement efforts as needed.
- Serves on intra and inter-agency task forces/committees as appropriate.
- Represents the agency at statewide programs as requested by the Program Asst. Dir.
- Actively pursues development of professional competencies related to job performance.
- Performs other duties as assigned by Program Assistant Director.
Scope of Position:
Average range: Supervision of 3 service coordinators, oversight of supervisor and 60 consumers, oversight of 6 COS’

Minimum:
EDUCATION and/or EXPERIENCE:
A Bachelor’s degree in psychology or a related field, or Associate’s degree and 3 years of experience in developmental services. Computer literacy, a valid Driver’s license and car insurance.
Preferred:
Experience in the file of Intellectual Disabilities. Experience in positions of leadership, supervisory teaching and training. Strong written and verbal communication skills. Good math and reasoning skills. Ability to listen effectively.
