Care Manager

For more than 25 years, we have been helping adults with severe mental illness and children with serious emotional challenges live quality lives in the community with ongoing support and treatment. We are the only agency in the state whose sole focus is persons with serious mental illness and children with serious emotional disorders who also struggle to survive at or below the poverty level. We are a strong team of caring professionals who want to influence, inspire, and impact lives through quality care, and we are currently looking for Care Managers for both adults and children to join our team. WHERE ARE WE?

Nashville, Cleveland, Antioch, Dickson, Chattanooga, Clarksville, Cookeville, Murfreesboro, Gallatin, and Columbia

• Assist with referral and linkage to community resources and advocating for consumers whenever necessary.
• Ensure consumers are seen upon discharge from the hospital and Diversion Services within Agency standards.
• Ensure consumer’s daily psychiatric, medical, and environmental needs are planned for and coordinated.
• Ensure efficient transition of consumers when needed within the Agency and the community.
• Meet regularly with Care Management Supervisor (individual and group) and/or Care Team
• Facilitate education of consumer and/or involved collaterals to psychiatric and medical diagnosis and appropriate treatment options.
• Respond to urgent/crisis consumer needs as appropriate to position with assistance of supervisor, crisis services, and clinic staff by developing plans and interventions
• Develop and maintain relationships within family unit and friends.
• Ensure timely and appropriate documentation as required by the Agency.
• Develop and maintain community relations through prompt follow through with contacts.
