This position will be based at the new and innovative acute care behavioral health facility, currently under construction and slated to open in the fall of 2023. SummitStone has the privilege of partnering closely with Larimer County on this venture. This voter-approved facility will provide licensed, equitable and culturally relevant care and treatment to individuals in crisis with mental health and/or substance use disorders. This facility will serve anyone residing in Larimer County in need of urgent intervention. As a Case Manager, within our Crisis Residential, the case manager provides services to individuals experiencing a behavioral health crisis. Duties include milieu management, brief counseling interventions, psycho-education and activities groups, and treatment planning within a Crisis Stabilization Unit setting. The position interacts extensively with a large interdisciplinary team including RN's, Psychiatrists, Nurse Practitioners, Mental Health Therapists, and Psychologists.

• Required: Bachelor's degree in behavioral science; Work experience with people severe and persistent mental illness/substance abuse issues or individuals with similar human-service needs; experience with Word, Excel, and Outlook. Valid Colorado driver's license with ability to be insured by SHP.
• Ability to pass a pre-employment drug screening for all federally illegal substances, including THC.

• Preferred: Bilingual/Bi-cultural; Trauma Informed Care experience. CAS certification.

Based on a 40-hr. work week. Additional program differentials may increase your base wage range.

• Between $17.94/HR up to $24.62/HR.
