Case Management Supervisor - Outreach

Schedule: Full Time - 40 hrs/week - Benefits Eligible!
Pay Rate: $21 / hour

This position assists the Director in providing daily program oversight including management of program staff that provides Housing case management and Outreach case management services to unsheltered individuals and/or families in Salt Lake County. Assist the Director to ensure that the program is working in accordance to agency and contract standards.

Essential Duties:
Staff Management

  • Provide training for staff on best practices in working with unsheltered and homeless population, community triage and housing process and access and referral to community resources to further the goal of making homelessness brief, rare and non-reoccurring.Provide direct supervision, guidance and support to Homeless Outreach Staff. Implement effective on-boarding training, program orientation for all new hires, and maintain/update training materials.
  • Participate in hiring, performance evaluations and disciplinary processes of direct reports.
  • Support staff development by facilitating training on a variety of best/evidenced based topics such as Stages of Change, Housing First, Motivational Interviewing etc. that could enhance the work staff do with clients.
  • Conduct and participate in staff meetings and training to promote consistent delivery of services. Coordinate bimonthly Care Coordination learning opportunities, presentations and field trips, identifying learning opportunities for staff, areas for improvement, and service needs.
  • Maintain staff schedule, meet with direct reports weekly to provide support and feedback for on-going development and thoroughly document meetings.
  • Monitor and ensure required staff coverage for program efficiency and staff safety.
  • Maintain regular communication with Housing Connect staff and all other stakeholders, and develop a good working relationship prioritizing communication and collaboration.
  • Maintain and oversee data collection, grant reports, grant reports, and grant compliance timely and accurately.
  • Attends community meetings as needed to enhance the work of the outreach team and/or continuum of homeless services.
  • Continuously monitor homeless services program strategies and work with the Program Director to ensure service delivery is effective and that clients are progressing towards safe and stable housing options.
Client Services

  • Evaluate unsheltered individuals for appropriate linkage to housing, health and other resources to help resolve homelessness and referrals to community partners.
  • Assist team as needed in actively engage in providing direct client service with those experiencing unsheltered homelessness.
  • Model De-escalation skills and Trauma informed practices for staff when managing difficult client behaviors.
  • Ensure a safe environment for staff and clients by listening to client and staff concerns, evaluating our processes and procedures to make any necessary adjustments to service delivery.
  • Utilize appropriate licensing, agency and contract requirements to guide daily program operations and program development.
  • Maintain and foster positive relationships with community entities, Homeless resource centers, co-workers, government entities, agency donors and emergency response services.
  • Attend Clinical Staffing regularly in order to provide insight, receive feedback, and support staff.
  • Communicate and address concerns or incidences with Program Director in a timely manner, being receptive to feedback or any direction provided.
  • Perform other functions as necessary or assigned.
Qualifications

  • Direct experience with individuals experiencing homelessness.
  • 2 years supervisory and administrative experience in a human service setting.
  • A Bachelor's degree, in a social services field preferred or any equivalent combination of education, experience, and training.
  • Ability to learn and utilize software programs used by VOA.
  • Knowledge and understanding of Motivational Interviewing techniques, Stages of Change models, Trauma Informed Care principles preferred.
  • Ability to uphold professional boundaries, agency policies and procedures, and interact in a professional manner with a diverse workforce, clients, and the public.
  • Ability to interact with clients and staff by demonstrating genuine empathy, concern and understanding of their experience while maintaining professional boundaries.
  • Ability to work in a professional manner with community members, governing officials, and community partners.
  • Ability to perform outreach in extreme weather conditions, including summer heat and winter snow; be willing and able to perform outreach on foot approximately five miles per day while wearing a backpack with supplies. Must be able to lift 25 lbs.
  • Must be at least 21 years of age, possess a current driver's license, have a good driving record and be insurable on the agency liability policy.
