Adaptive Home Health is looking for a Clinical Liaison to join a fast-growing team. We service patients throughout throughout the DFW metroplex and across North Texas.

We are an entirely nurse-driven organization. Our clinical staff are quality, experienced, well-paid nurses that are show up every day to care for patients because they know we care for them.

We’re looking for a self-driven individual to help manage, maintain, and grow our relationships with referral sources in the Fort Worth area to ensure that our good name continues to spread.

Experience/Education:

• Highschool Diploma/GED
• Minimum 3 years healthcare experience (Home Health preferred)
• Training provided
• Licensed RN preferred

Major Responsibilities:

• Gives high priority to compassionate patient care and patient satisfaction.
• Visit patients prior to discharge
• Establish a trusting relationship with patients
• Establish, maintain, and expand relationships with diverse referral sources
• Keep detailed records of marketing activities
• Identifies and resolves service issues with extreme emphasis on patient and referral-source satisfaction
• Mixed remote/in-person work environment

Qualifications:

• Understands and adheres to all external accreditation review standards, applicable state, local and Federal laws and / or regulations including maintaining patient confidentiality through abiding by HIPAA laws/regulations.
• Understands and adheres to all company policies and procedures.
• Takes initiative to present ideas / suggestions to management regarding process improvement.
• Ability to work independently is a MUST

Job Type: Full-time

Pay: $75,000.00 - $95,000.00 per year
