Detail of Key Job Responsibility Areas
1. Care Management Support
- Assist the Care Management department team in providing patient-centered, quality driven, culturally appropriate comprehensive services to patients and their families in compliance with agency policies and funder requirements.
- Schedule appointments on behalf of patients and facilitate patients appointment-keeping.
- Responsible for onboarding patients to services, assisting with completion of paperwork (intake forms, consents, etc.) and introducing them to team members involved with the program(s).
- Assess patient needs and assist with coordinating referrals to internal/external services to meet those needs.
- Build relationships with the patients and all members of the treatment team to support continuity of care and promote health.
- Support patients with adherence to treatment recommendations.
- Document each patient interaction comprehensively and concisely. All documentation will be submitted to supervisor within the designated time frame.
- Participate in case conferences with Clinic, Care Management, and other departments to discuss patient retention and engagement in treatment/care.
- Adhere to all government and funder regulations.
2. Outreach
- Responsible for being the organization’s point of contact for potential patients, demonstrating knowledge of and skill in speaking about the services Trillium offers.
- Supports the Care Manager to engage and retain patients in care.
- Demonstrate developing knowledge of community resources and community-based organizations/providers that may be serving potential patients.
- Responsible for conducting outreach and educating the community, health care providers/clinics, and other agencies regarding Trillium Health Services.
- Expected to conduct outreach in the community much of the time.
3. Advocacy
- Identify, arrange, coordinate, and advocate for patient services.
- Orient patients to services available within the agency and to the mission at Trillium Health.
- Advocate for patient access of entitlement programs, treatment facilities, and other needed community services.
- Promote patient access into internal agency services including, but not limited to support groups, housing services, Food Services, transportation, behavioral wellness, substance use treatment, medical care, and Harm Reduction services.
4. Miscellaneous
- Requires the ability and commitment to respect and support inclusiveness and diversity, including but not limited to individuals of different backgrounds, cultures, races, ages, sexual orientations, gender identities or expressions, transgender, experiences, opinions, etc.
- Requires individual demonstration of commitment to the One Trillium values, behaviors and business impacts and modeling them in the organization.
- Responsible for maintaining confidentiality of all patient, client, employee, protected and proprietary information.
- Must have access to a reliable vehicle and meet the agency driving policy requirements for this position, including a valid NYS driver’s license and proof of required automobile insurance for the vehicle used.
- Employees are accountable for meeting the performance standards of their departments and must participate as requested in compliance audits, process improvement and quality improvement plans.
- Other duties as assigned.
Competencies
Demonstrate developing knowledge of healthcare and related issues.- Attend conferences and professional development offerings to further knowledge and skills related to outreach, health disparities, social determinants of health, HIV/AIDS and other health issues.
- Requires the ability to work outside of normal business hours (Monday-Friday 8:30am-5pm) to include occasional evening and weekend hours to participate in outreach events/activities.
- Must be proficient in Microsoft Office; Word, Excel, Outlook and Explorer and have the ability to learn and navigate the HIT systems used.
- Excellent organizational and time management skills required.
Qualifications
Associates degree in health or human services plus one year of qualifying human services experience including case management or casework with persons who have HIV infection, a history of mental illness, homelessness or chemical dependence preferred; OR a High School diploma/GED and two years of qualifying experience. Computer proficiency is required. Fluency in Spanish and/or ASL preferred. Must be accustomed to diversity.
Physical Requirements
While performing the duties of this job the employee is required to stand, sit, walk, use hands to finger, handle, or feel; reach with hands and arms, talk and hear. Occasionally the employee must stoop, bend and lift or move up to 25 lbs. Specific vision abilities required include close vision, distance vision, peripheral vision, depth perception and ability to adjust focus.
