YOUR LIFE AS A DSP - PERSONAL CARE ASSISTANT

This habilitation position with our human services nonprofit is full-time. Our available shifts are currently 7 am - 3 pm. or 2 pm - 10 pm. Hours for our caregivers include nights, weekends, and holidays.

As a Direct Support Professional with our human services nonprofit, your days are filled with purpose and meaning as you make a positive impact on the lives of individuals with disabilities in your community. You start your day with a smile, knowing that you are about to embark on a fulfilling journey of helping others achieve their goals and gain independence.

Each day on our habilitation team, you provide direct supervision to individuals in accordance with their treatment plans. You are well-versed in various competencies, such as task analysis, life plan/staff action plan, goal implementation, job coaching/teaching, report writing, and behavioral interventions, as required by the NH and NY Programs. As a caregiver, you are skilled at observing behavior, skill levels, and areas of interest, all while providing objective information to formulate data-based recommendations.

You are a model DSP and facilitate the development of appropriate social skills, encouraging the establishment of social relationships. In this caregiver position, you help individuals become more independent by facilitating community-centered activities, job tasks, or residential activities. You coordinate schedules that reflect integrated opportunities, geared toward increasing their independence, community access, and awareness.

While working as a caregiver on our habilitation team, you are proactive in assisting with the development and revision of training plans, life plan/staff action objectives, and fading plans, as needed by the NH and NY Programs. You then implement approved training and fading plans, Life Plan/Staff Action objectives, and behavior plans, ensuring quality control standards are met in accordance with employer and volunteer expectations.

You are also responsible for maintaining documentation and preparing reports in approved formats and designated timelines. You ensure that documentation and authorization for dispensing medication are up to date, as applicable. You may also provide transportation to individuals with disabilities in your personal vehicle or agency van for program activities and appointments.

You provide training in various life skills, such as self-care, money management, cooking, social interactions, transportation systems, and more, as approved by the treatment team. You assist individuals who have disabilities with personal care needs when necessary, and communicate pertinent information with the team and support system to ensure that individuals receive the maximal benefit from the services offered.

Safety is a top priority for you as a caregiver, and you demonstrate appropriate judgment and understanding of client safety while supporting individuals. You follow through with supervisory requests and seek supervision when unclear about expectations, ensuring that you provide the best possible care to those you serve.

With our habilitation team, you also perform routine household tasks, such as meal preparation, housekeeping, and outdoor maintenance, as applicable. Additionally, you participate in available training opportunities to expand your knowledge of treatment methods, as 20 hours of training are required per year. You are committed to following all policies and procedures outlined by the department and agency, and you are always ready to perform additional duties as requested to fulfill our human services nonprofit's mission. WHAT WE NEED FROM YOU AS A DSP - PERSONAL CARE ASSISTANT

  • High school diploma or equivalent
  • 2 years work experience preferred
  • Ability to become medication certified (AMAP) and successfully complete Easterseals NY Rochester required training and obtaining the associated certificates
  • Valid driver's license
