What you will do

Responsible for the management and coordination of the Facility. Ensures continuity, cost effectiveness, and quality of operations.

Responsibilities

  • Operates in the Dyad Leadership Model with the Medical Director to oversee the direction, operations, functionality, performance, and operating culture of an identified area of collective work.
  • Responsible for coordinating and assuring a high standard of quality care consistent with each profession on the interdisciplinary team and Partners In Care mission and core values.
  • Communicates organization information and liaises between senior leadership and the Hospice House staff.
  • Facilitates communications and works effectively between Facility staff, patients/families/caregivers, physicians, vendors, other departments, external referral sources, and professionals as appropriate.
  • Provides leadership, problem-solving, and management decision-making.
  • Monitors documentation and implementation of physical, psychosocial, and spiritual interventions for the patient/family/caregiver.
  • Reviews staffing and scheduling and communicates information with Clinical Operations Director.
  • Evaluates the quality of care delivered by professional staff, as evidenced by clinical assessments and medical record review.
  • Serves as a liaison with other departments to resolve problems and improve efficiency and effectiveness.
  • Assures implementation and compliance with all policies and procedures, licensing and certification rules, regulations, and guidelines.
  • Responsible for financial management for the Facility, including assistance with budget preparation, expense approvals, and being responsive to all financial issues.
  • Responsible for managing licensing and Medicare certification compliance, including all rules and regulations.
  • Responsible for maintaining compliance with Health Information Management (HIM).
  • Responsible for ensuring regulatory rules and regulations are being followed as set by the Oregon Health Authority, including but not limited to the “Hospital Nurse Staffing Committee” and “Specialty Inpatient Care Facilities.”
  • Completes employee evaluations in a timely manner.
  • Participates in annual budget preparation with Clinical Operations Director and Chief Financial Officer
  • Provides decision-making regarding contract services as appropriate.
  • Articulates patient needs transferring between levels of care to staff and promotes continuity of care.
  • Facilitates patient care and quality management activities by assuring staff participation and Facility representation in focus groups; follows up on surveys and grievances; performs medical record and utilization review audits.
  • Actively participates in Quality Assurance, Utilization Review, and Risk Management activities.
  • Actively participates in staff meetings, department meetings, management meetings, in-services, competency training, and other related activities.
  • Participates in provision of educational and public relations activities to the community and professionals.
  • Supervises all Facility staff; serves as preceptor; provides ongoing education to patient care staff.
  • Presents to Clinical Operations Director ongoing plans for improved efficiency and effectiveness of care and services.
  • Provides leadership and formal/informal education to staff through in-services, case conferences, and group discussions on Hospice philosophy, operations, policy and procedures, and problem-solving.
  • Provides a safe environment for patients, guests, visitors, and employees.
  • Adheres to all policies, procedures, and regulations regarding patient care, conduct, safety, fire, security, and risk management.
  • Carries out duties and responsibilities of the position in a manner that exemplifies excellent customer service.
  • Maintain relationships with co-workers and staff based on trust, support, and growth.

  • Graduate of an accredited school of nursing with current licensure to practice professional nursing in the State of Oregon
  • Bachelor of Science degree in nursing
  • Minimum of five years of clinical experience and 2 years of supervisory experience
  • Current automobile insurance required
  • Valid Oregon driver’s license and reliable transportation required
  • Current CPR card required
  • Demonstrates an ability to work with other healthcare clinicians, develop a plan of care, knowledge of third-party reimbursement, and negotiate payment rates.
  • Self-directed and able to work with minimal supervision.
  • Demonstrates excellent observation verbal and written communication skills.
