General Description
Niagara County Community College is seeking a Part Time Case Manager. This is a grant-funded position reporting to a Program Director within the Liberty Partnerships Program. This position is responsible for providing services to students in grades 5-12 who are enrolled in partner school districts including conducting outreach, administering program assessments, planning and implementation of activities, providing academic assistance, and the overall evaluation of students’ needs.
Hours fall between 8:00-4:30 Monday- Thursday. This position includes paid planning time. Remuneration is $25 per hour.

Typical Work Activities
The Case Manager is responsible for providing program services to all students enrolled in the Liberty Partnerships Program. Responsibilities include, but are not limited to:
1. Administer program assessments. Use assessment data to drive programming to meet the needs of the student population.
2. Provide academic instruction to assist students with homework completion in the various content subject areas.
3. Design and implement lessons, activities, and trips for students according to grant guidelines. These activities will include social and life skills improvement, academic assistance, community service, employment preparation, college readiness, enrichment or cultural activities, mentoring, civic service, leadership, sports and wellness, social and emotional skill development.
4. Plan, develop, and conduct workshops including but not limited to financial literacy, career and college exploration, interviewing skills, resume building, effective communication, setting and achieving goals.
5. Serve as a student advocate. Enhance collaborative relationships to maximize the student’s and family’s ability to access supportive services.
6. Will work closely with partner school counselors to identify and case manage Liberty students.
7. Prepare and make presentations to groups.
8. Conduct home visits for new participants, for material drop off, and maintain ongoing home contact as needed.

Essential Functions
Must be able to relate to a diverse student population and academic personnel; must be able to manage multiple functions; must have effective oral and written communication skills; must work well independently and as a team member; must have experience with Microsoft Office, Microsoft Excel, spreadsheets, and database management; must possess good organizational and interpersonal skills; must be able to travel between multiple Niagara County school districts, must be able to physically perform the duties of the position.
Required Qualifications
- Bachelor’s Degree
- A minimum of three (3) years of full time or equivalent part-time paid related professional experience working in an educational setting.
- Must be able to prepare and make presentations to groups.
- Must hold a valid driver’s license and have reliable transportation in order to conduct home visits.
Desirable Qualifications
Master’s Degree in Education or related field.
Three (3) years full-time or equivalent part-time paid, professional experience working in an educational setting. Experience working with students identified as “at risk”, low-income, or first-generation students.
