About PACE:

Philanthropists Gary and Mary West founded West Health with the mission of enabling seniors to successfully age in place with access to high-quality, affordable health and support services that preserve and protect their dignity, quality of life, and independence. The Gary and Mary West PACE (Program of All-inclusive Care for the Elderly), affiliated with West Health (www.westhealth.org), is a new non-profit that delivers medical, social, and behavioral services to the frail elderly to the North County of San Diego. This PACE center will be entrepreneurial in nature and will focus on innovations and bringing new processes to the traditional PACE environment.

Position Summary:

Under the leadership of the Center Director, the Personal Care Supervisor is responsible for the planning and monitoring of personal care services in-home and in the PACE Day Center, the hiring and ongoing supervision of the Home Care Coordinator(s) and Personal Care Attendants.

Essential Functions:

  • Participate as a member of the Interdisciplinary Team (IDT) and in initial, semi-annual, unscheduled, and annual assessments.
  • Participate in morning updates and report changes in participants' status to appropriate staff regularly.
  • Perform homecare assessments for Service Delivery Requests (SDR) and update in the EMR per regulatory guidelines, as required.
  • Conduct initial prospect intake, meeting with family members and others, in conjunction with the IDT. Coordinate ongoing family meetings, as needed.
  • Ensure that schedules for personal care in the home and day center are accurate and that participants are allocated enough time/services to meet their individual medical needs.
  • Provide back-up for HCC (Home Care Coordinator) when necessary.
  • Assist in developing and updating policies and procedures pertaining to personal care services in-home and at the day center.
  • Plan for adequate staffing of in-home workers to meet participants' needs and anticipated needs of incoming prospects.
  • Supervise Personal Care Attendants and Homecare Coordinator(s).
  • Coordinate service requests/authorizations with contracted home care agencies; Monitor delivery and quality of services provided by home care contractors.
  • Assure that documentation for provision of personal care in the home is accurate, current, and entered in the EMR timely.
  • Facilitate follow-up investigations and documentation for complaints and grievances relating to personal care in the home and the day center, according to the GMWP grievance policies and procedures.
  • Provide accurate and timely homecare utilization data to the Center Director regularly.
  • Participate in the development and implementation of Quality Improvement activities.
  • Participate in the regulatory surveys, as appropriate.
  • Maintain confidentiality regarding participant, staff, contractor, and organizational information.

Non-Essential Job Functions:

  • Attend and participate in staff meetings, in-services, projects, and committees as assigned.
  • Adhere to and support the center's policies, practices, and procedures.
  • Accept assigned duties in a cooperative manner and perform all other related duties as assigned.
  • Be flexible in schedule of hours worked.
  • Required use of personal vehicle.

Education and Certification:

  • Current California LVN license preferred
  • Current Basic Life Support (BLS)/First Aide Certification required

Skills & Experience:

  • Minimum of two (2) years of demonstrated successful experience in home care including one year of in-home care management experience.
  • Minimum of one (1) year of documented experience working with a frail or elderly population.
  • Experience in the provision of person-centered care, including assessment skills, personal care, and health education.
  • Working knowledge of physical, mental, spiritual, and social needs of the frail elderly and their families.
  • Effective management skills.
  • Knowledge of utilization review, quality improvement and cost containment systems.
  • Ability to work with ethnically diverse populations.
  • Ability to apply creative problem-solving skills to the complex issues in home care.
  • Ability to work with interdisciplinary teams.
  • Strong organizational skills, flexible, resourceful, and energetic.
  • Effective oral and written communication skills.

Medical Clearance:

Employees must have medical clearance for communicable diseases and up-to-date immunizations before having direct participant contact. Job offers are contingent upon a successful pre-employment drug screen, background check, and physical assessment. Up to date COVID vaccination is required of all employees.
