Responsible for providing, under the direction of a Physical Therapist, therapeutic interventions in an interdisciplinary environment consistent with the positions qualifications, professional practices, and ethical standards. Demonstrates accountability for and contributes to program development, quality improvement, problem solving, and productivity enhancement in a flexible interdisciplinary environment.

RESPONSIBILITIES / TASKS:

• Provides age-appropriate therapeutic interventions as directed by the physical therapist, home health, and outpatient care.
• Evaluates resident response to treatment and provide feedback to the rest of the team.
• Documents the course of resident care including progress made and continuing need for treatment.
• Participates in survey preparation activities.
• Collaborates with the physical therapist to develop new methods of delivering education to residents and family.
• Assists the physical therapist in providing team members with in-service/cross training on physical therapy knowledge and skill areas, as needed.
• Reports and complaints and/or grievances made by the residents to the Director of Therapy. Reports incidents or suspected incidents of resident abuse to the Director of Therapy and/or the Executive Director immediately.
• Complies with all resident rights per policy.
• Maintains professional/ethical competencies and proficiencies for job responsibilities.
• Communicates effectively with discharge planners, physicians, nursing staff, therapists, and/or interdisciplinary team regarding patient care, including those from other environments (hospitals, long-term care, therapy, etc.)
• Appropriately and effectively completes documentation including evaluations, daily progress notes, weekly summaries, or monthly reports as required
• Uses professional judgement to ensure safety for residents, others, and self at all times
• Perform other duties as assigned and/or needed

EDUCATION & EXPERIENCE:

• Graduate of an APTA approved associate degree program or equivalent training that meets all state practice requirements and hold an applicable, unrestricted license to practice.
• Minimum of 2 years of experience in a healthcare/senior living or long-term care environment working with an adult/geriatric population is preferred.
• Intermediate computer skills (MS Word, Excel, PowerPoint, and Outlook) and technical aptitude
• Working knowledge of Medicare and other payer source billing
• Full knowledge of Resident’s Rights
• Advanced written and verbal communication skills; able to work/communicate effectively with residents, staff, and others
• Advanced interpersonal skills to communicate in a courteous and respectful manner

CERTIFICATIONS, LICENSES, and OTHER SPECIAL REQUIREMENTS:

• Possession and maintenance of a license in good standing (no restrictions) with the applicable state as a Physical Therapy Assistant.
• Possession and maintenance of a valid driver license issued by the state of personal residence
• CPR certification (hands-on and in-person skills assessment)
• Must have compassion for and desire to work with the elderly.
• Must be COVID-19 Vaccinated with booster.
