THE POSITION
The City of Long Beach, Department of Health and Human Services is recruiting for a, full-time Community Program Specialist IV, in the Community Health Bureau. Reporting to the Community Health Bureau Manager, the Population Health Management Specialist supports with landscape analysis, curriculum development/adaptation for cultural responsiveness, coordinates with existing outreach teams to leverage community access, and performs monitoring and evaluation activities. This position is located at Main Health, Long Beach, CA 90807.
EXAMPLES OF DUTIES
- Analyzes and interprets data to develop conclusions and makes recommendations that will address disproportionate impact chronic conditions on multi-ethnic community;
- Uses evidence-based and/or evidence informed strategies to tailor and adapt behavior change curriculum for multi-ethnic populations living in Long Beach for improvement in service delivery, performance, and attainment of program goals;
- Identifies, establishes, and maintains relationships and serves as liaison to task forces, committees, stakeholders, clients, businesses, and other agencies related to grant funded programs, projects, and/or services that pertain to Diabetes, Obesity, Cardiovascular, and other chronic diseases;
- Coordinates and/or provides professional support in the development, implementation, administration, and/or management of a variety of programs, projects, and/or services;
- Interprets complex local, state, and/or federal regulations as it relates to the program goals, program directives, and ensures compliance;
- Designs processes for intervention feasibility and evaluation;
- Oversees fiscal requirements, prepares program budget documents, and reports;
- Develops and implements marketing/communication strategies for programs;
- Prepares and presents program and service information at public meetings and events;
- Creates, reviews, or maintains client and program files, performance benchmarks, and other reports;
- Prepares reports, applications, evaluations, program enrollment statements, and other correspondence;
- Coordinates public meetings and events to promote programs and services;
- May train and supervise the activities of subordinates;
- Monitors and facilitates participant’s involvement in programs;
- Participate in fund development opportunities to sustain Chronic Disease-related efforts;
- Engage, collaborate, and coordinate with other City and community stakeholders to ensure equitable opportunities for health and wellness in our City relative to chronic disease;
- Develops tools and customized plans to be used by clients; and
- Performs other related duties as assigned.
REQUIREMENTS TO FILE
- Bachelor's Degree in Public Health, Statistics, Public Administration, or closely related field.Professional experience coordinating grant programs, projects, and services may be substituted for the required education on a year-for-year basis.
Copy of degree or transcripts denoting degree conferment required.
- Three (3) years full-time professional experience working in finance, grants administration, and/or procurement.A Master’s Degree in Public Health, Statistics, Public Administration, or closely related field from an accredited college or university may be substituted for up to one (1) year of the required professional experience.
- Valid motor vehicle operator license. (Required to obtain a California motor vehicle operator license within 6 months if out of state. DMV K4 driver license information report printout required during onboarding for California residents.)

DESIRABLE QUALIFICATIONS
- Willingness and ability to work some evenings and weekends as required
- Experience with grant management, finance, procurement, and/or accounting
- Experience with fiscal audits
- Experience with the administration of public health, communicable disease, or emergency management programming
- Bilingual skills (Khmer, Spanish, Tagalog, and/or Vietnamese)

- Ability to work cohesively in teams by consistently exercising tact, good judgment, and a proactive, problem-solving focused style.
- Excellent interpersonal, written, and verbal communication skills
- Strong organizational skills, ability to work independently and be flexible.
- Proficiency in the use of computers, including email, internet, Teams, and Microsoft Office, specifically Excel.
