Full Job Description

Under the direction of the DON the home visit RN/LPN will manage and oversee a caseload of clients to deliver comfort and excellent care as required.

Responsibilities and Duties

• Provide skilled nursing care to pediatric and adult patients in a home setting Implement, develop, and document the plan of care to ensure quality and continuity of care for each patient
• Provide effective communication to patient/family, team members, and other healthcare professionals
• Implement plan of care and treatment using sound judgment and critical thinking Provide accurate, detailed, and timely documentation consistent with the plan of care and the nursing process
• Administer oral, IM, Subcut prescribed medications, also treatments, and therapies
• Participate in staff meetings to exchange ideas and information as it relates to the quality initiatives.

Qualifications and Skills

• Current Registered Nurse in the state of MA
• Minimum 1+ year of clinical home care experience /New grads are WELCOME AND WILL BE TRAINED
• Exceptional listening and analytical skills required. Excellent attention to detail.
• Work positively and productively as part of a team. Work flexible hours when needed.
• must demonstrate excellent notes and documentation skills.
• Experience in Home Health Care Knowledge of HIPAA regulations Work independently.
• Maintain confidentiality.
• Current CPR accredited by American Heart Association and/or American Red Cross (certificate must be healthcare provider or healthcare professional).
• Valid MA Drivers license

Physical Setting:

Home Health visits Education: RN license Job Type: Full-time

Salary: $80,000.00 - $90,000.00 per year
