JOB SUMMARY: The Host Home Program Manager is responsible for the day-to-day operations of DDM’s Host Home Program for adults with developmental disabilities, and for general supervision of host home providers and personnel for Host Home Program. This position involves a high level of community engagement and accountability. This is DDM’s first host home program. As such this position will have a strong program development, research and evaluation component. The beginning phase of this pilot program will establish an operational program to include documentation requirements and initial training that are inclusive of DBHDD policies as well as the COMP Waiver manual.

The Goal:Start host home provider recruitment from Metro Atlanta to Rome, Ga and other areas surrounding current group homes in the first 2 months upon hire. Recruitment of a minimum of 8 host Homes providers per year.

JOB REQUIREMENTS:

There is a minimum requirement of a bachelor’s degree in the broad field of human services (such as social work, psychology, teaching, healthcare, or a similar related field.) Must be able to qualify as a Developmental Disabilities Professional (DDP). Master’s degree is preferred. Experience required in a position related to Host Home Program Recruitment and Program Management.

· Hours: 40 hours per week, including some evening and weekend hours

· Location: Office will be based in Norcross, Ga, but position will be part of DDM’s mobile workforce.

· Compensation: Commensurate with qualifications and experience

OUTCOMES OF THE POSITION:

· Recruitment of a minimum of 8 host Homes providers per year

· Daily functions of the program operate efficiently and safely, basic needs of the residents/core team members are met.

· Program is in compliance with all governing rules, regulations and policies.

· Documentation pertaining to host home recruitment is completed, compiled accurately and on a timely basis.

· Host Home applications to submitted to DBHDD are complete and regular check-ins with to DBHDD are required to ensure timely approval.

Pay: $24.00 - $28.00 per hour
