We are currently seeking qualified candidates for a full time Quality Assurance Coordinator position.

Job Description:

A Quality Assurance Nurse is responsible for assuring quality control of all health services provided by the Agency and for reviewing documentation for appropriateness of care.

Responsibilities:

Maintain Divine Compassion Hospice compliance with ACHC standards and Medicare regulations related to the provision of Hospice services.

Measures, monitors and reports performance to ensure program goals are being attained, and follows the continuous quality improvement cycle to improve outcome.

Create and utilize a standard audit finding report for communicating the results of the audits in a clear concise manner.

Monitors client clinical records concurrently and retrospectively, utilizing established outcome criteria (i.e., plan of care, goal, and objectives).

Reviews clinician documentation for accuracy. Communicates regularly with clinical staff and provides ongoing education. Assists the Chief Operating Officer, Chief Clinical Officer and or Director of Quality Improvement in implementing the agency's Quality Assurance and Improvement Programs.

Performance Improvement Coordinator

Coordinating with other departments such as human resources or communications to ensure that all policies and procedures are followed

Establishing goals and metrics for performance improvement initiatives, such as identifying areas for improvement in the company’s safety record or employee satisfaction levels

Developing training programs in collaboration with other staff members, including instructional designers and instructors

Conducting interviews with employees whose performance may be impacted by the training program, such as managers whose direct reports will be participating in the program.

Conducting surveys of employees to determine their opinions about workplace issues that may affect performance

Creating reports documenting the findings from surveys or other research activities

Conducting exit interviews with employees who leave the company to determine the reasons for turnover

Reviewing employee performance evaluations for accuracy, consistency, and compliance with company policy

Meeting regularly with supervisors to discuss employees’ progress and offer advice about how to improve performance

Qualifications:

  • Licensed LVN in the State of Nevada
  • 5+ year's required in Quality Assurance
  • Experience in reviewing nurses' notes for completeness, compliance with physicians' orders, authorized services, regulations required
  • Experience in reviewing admission packets required
  • Experience required in reviewing all POC and recertification for: accuracy, order updates, frequency updates, goal updates, medication profiles review, format and wording, diagnoses update, C.N.A. care plan
  • Conducts one-on-one in-services for documentation problems as needed
  • Experience with hospice, home health or any experience in the healthcare field required
  • Ability to travel
  • Effective written and verbal communication
  • Clean background and drug screen

Job Type: Full-time

Pay: $25.00 - $30.00 per hour
