We are currently seeking full-time Transition Specialists in North Carolina for a brand new, exciting program called “The Bridges”.
These are remote positions where you can work from the comfort of your home office.

In this role as Transition Specialist, you will be a part of TRANSFORMING the way that we do residential services. You will work directly with the parent/family of youth who are currently in residential care at New Hope Carolinas in Rock Hill, SC. BUT – you’ll be working with them in
their home/community and preparing them to receive their child back successfully. This could be revolutionary to the outcomes for kids in residential care as so often their parents/family who are still living in the home receive zero support. This is a brand new, exciting program and a huge step forward in the mental health system. If that excites you, come be a part of it.
To be successful in this role, you will display a compassionate nature, empathy, and discretion in your work. You should have excellent communication and time management skills and be able to develop and maintain relationships with clients, supervisors, and colleagues. You must be a QP in North Carolina to qualify for this role. Previous experience in community-based services, case management and mental health services is preferred.
Responsibilities include:
- Meeting with parents/families in their home/community weekly
- Case management duties to ensure the family has the services and supports they need
- Counseling/coaching/supporting the parents so they are involved in their child’s residential treatment
- Work collaboratively with stakeholders and other professionals involved in the family
- Participate in CFTs
- Build parental skills and capacity
Qualifications include:
- Bachelor or Associate Degree in a behavioral health, Early Childhood Education, Psychology, Special Education, or related field preferred.
- You must be at least 21 years old
- Two (2) years of experience in a behavioral health environment and able to qualify as a QP in North Carolina.
- Excellent observational skills and attention to detail
- Excellent verbal communication skills
- Ability to remain calm in high pressure situations
- Must pass a background check
