SUMMARY: A Registered Professional or Licensed Practical Nurse employed to evaluate, plan, implement and supervise the 24-hour delivery of resident care for a Nursing Unit.

REPORTS TO: Director of Nursing Services


ESSENTIAL FUNCTIONS:


1. Responsible for all care delivered on the Nursing Unit, seven days a week and 24-hours a day.

2. Directs the day-to-day functions of the Nursing Unit in accordance with current federal, state and local standards governing long-term care facilities.

3. Maintains and supervises the operational aspects of the Nursing Unit in collaboration with Nursing Administration.

4. Participates in developing, maintaining, and periodically updating nursing policies, procedures, reference materials, manuals, objectives and philosophies.

5. Coordinates nursing services with all other disciplines. Ensures completeness of Monthly/Weekly/Daily weights and coordinates findings with Dietician.

6. Assures the initiation and accurate completion of Minimum Data Set and Care Planning process including accurate completion of MDS summary, quarterly/annual assessment, psychotropic summary, and care planning IN A TIMELY MANNER. Observe ARD and deadlines.

7. Makes independent decisions concerning nursing care in accordance with Nursing Standards of Practice and Federal, State, local and Facility standards, and regulations.

8. Delegates to the staff nurses the administrative authority, responsibility necessary to perform their assigned duties in accordance with their respective position descriptions.

9. Makes daily rounds of the Nursing Unit to visit residents, and to observe and evaluate the physical-emotional state of each resident.

10. Has knowledge of each resident’s condition at all times.

11. Ensures that nursing personnel are performing their work assignments in accordance with Nursing Standards of Practice, position descriptions, and specific job duties. Monitors and oversees completion of C.N.A. documentation (ADL’s).

12. Maintains and updates a written schedule of staff work hours, and assignments on a 24-hour basis following federal and state mandated staffing requirements. Evaluates assignment distribution periodically. Evaluates assignment accuracy.

13. Assures that the physician has been alerted to changes in resident’s condition, lab work, etc. Obtains orders as indicated. Ensures resident/responsible party is notified of resident changes in accordance with state and federal regulations.

14. Responsible for the proper transcription and execution of physician’s orders, accurate documentation, maintenance of the clinical record completeness.

15. Keeps the Director of Nursing Services informed of the status of all residents, nursing personnel, and other related matters. Acts on behalf of the Director of Nursing Services in her/his absence.

16. Assists in the professional growth of the nursing staff, which includes but is not limited to in-service education, evaluations, and professional development. Coordinates the completion of all documentation necessary to support the above.

17. Participates in developing, planning, implementing, conducting, and scheduling orientation, training, in-service educational activities, employee performance evaluation and disciplinary action for nursing services personnel.

18. Coordinates all services rendered to the resident to include the evaluation and development of the resident’s ongoing plan of care. Consults with residents, significant others, and members of the Interdisciplinary Care Plan Team members to determine the needs of the resident.

19. Attends and participates in various committee (i.e., Safety, Infection Control, Policy Development, Compliance & Ethics, etc.) as assigned. Supervises the nursing staff in compliance of new programs/procedures implemented in the facility.

20. Assists in maintaining facility compliance with OSHA regulations.

22. Must practice universal precautions as advocated by the Centers for Disease Control. Ensures compliance with Infection Control Guideline including 2 step Tuberculin skin Test within 30 days of admission: review and document immunization history, consents and vaccination of resident as appropriate (Flu vaccine and Pneumonia vaccine).

23. Ensures resident’s confidentiality and privacy is maintained and that each resident is treated with dignity and respect at all times.

24. Immediately reports allegations or suspicions of abuse to Administration. Participates in investigation of all instances of actual or suspected verbal or physical abuse of the resident, and reports findings to Director of Nursing, Administrator, and Social Worker.

Investigates complaints made by either resident or responsible party in collaboration with the Director of Nursing Services and Social Worker. Collects statements from staff in order to complete incident investigation for bruise/skin tears of unknown origin.


25. Coordinates with DON/ADON weekly wound rounds, new treatments and updating of resident care plan.


26. Ensures compliance of pharmacy consultant's recommendation in a timely manner. Ensure compliance with medication storage i.e., clean / organized med room, clean/organized carts.


27. Responsible for completeness and accuracy of information of all admission and readmission including obtaining weights, delivery of medication, follow up consultation, devices, and care planning; ensure accuracy of medical records (i.e., care plan; immunization; assignment) for any resident transitioning to long term care from sub-acute rehab unit.


28. Supervises the main dining room to ensure residents are served the appropriate meal and properly assisted with feeding.


28. Performs floor duty in the event of staffing shortage. Participates in On-call Rotation as part of Nursing Administration duties to cover staffing needs.


29. Ensure resident follow up appointments/consults are in-place.


30. Ensure compliance of Nurses/ CNAs in Infection Control Precautions including compliance with use of PPE, providing resident, staff and visitor education and initiation and completion of Care Plan.


31. Conducts regular rounds at a minimum 3 x per shift.


32. Ensure accurate staffing prior to the start of the shift and the next shift.


33. Collects and reviews 24- hr. report for all units.


34. Assist Interdisciplinary Team in coordinating resident discharged including use of oxygen, wound care, BIPAP/CPAP, medication reconciliation.


35. Review and ensure Incident Reports are thoroughly completed to include statements from all responsible staff members.


36. Member of Inter-disciplinary team and participates in family meeting ensuring accuracy and timeliness of care plan and MDS summary in compliance with the State / Federal regulation.


KNOWLEDGE, SKILLS, AND ABLILITIES:


1. Expert knowledge of general nursing theory and practice.

2. Thorough knowledge of principles, methods, and techniques involved in performing general nursing services.

3. Knowledge of geriatric and restorative nursing theory and practice.

4. Knowledge of managerial techniques, labor relations, and personnel policies and implementation of same is helpful.

5. Knowledge of Medicare and Medicaid standards.

6. Knowledge of Federal and State Regulations.

7. Excellent leadership, supervisory, organizational and communication skills.

8. Must be able to relate well to staff, residents, family members, and the general public.

9. Must be able to read, write, and speak the English language.


EDUCATION AND EXPERIENCE:


1. Must be a graduate from an accredited school of nursing.

2. Possess a current license to practice as a Registered Professional Nurse or Licensed Practical Nurse in the State of New Jersey.

3. 1-2 years supervisory experience in a long-term care facility preferred.


PHYSICAL REQUIREMENTS:


1. Must be able to sit, stand, bend, turn, reach, stoop and crouch.

2. Must be able to move about the facility.

3. Must be able to lift residents to assist in transfers from bed and to wheelchair and back.

4. Must be able to push and pull.

5. Must possess visual and aural acuity to detect changes in resident condition and/or employee status.


WORKING CONDITIONS:


1. May be exposed to blood or other potentially infectious fluids or materials that require the use of

Universal Precautions.

2. May be exposed to unpleasant odors.

3. May be exposed to verbally and/or physically abusive residents.
